TRAINING COURSE FEE

ADMISSION REQUIREMENTS

A candidate to become admitted must fulfill the following requirements:

For Members

· Be a registered, active member of a REDES® or MIM-EW® member-association, member-society, member-consortium, or member-fellowship.

· Be at least 18 years of age or more.

· Be a citizen or national of Peru or a foreign country.

· Be physically and mentally health and fit.

· Be academically prepared with certified training and certified experience related with HICS.

For Non-Members

  • Be a registered, active member of a healthcare/hospital institution, disaster crisis response organization or related.
  • Be at least 18 years of age or more.
  • Be a citizen or national of Peru or a foreign country.
  • Be physically and mentally health and fit.
  • Be academically prepared with certified training and certified experience related with HICS, ICS in healthcare/hospital, healthcare engineering, healthcare/hospital/health services management, or medical/health services for disaster crisis response in Peru and/or abroad.

FOR MEMBERS

  • A candidate must fulfill downloadable or online version of the application form.

FOR NON-MEMBERS

  • A prospective candidate must send an email with the following documents in PDF format:
    1. Handwritten Cover Letter in English, dated and signed indicating the reasons to become a candidate.
    2. Curriculum Vitae typed and dated in English, with full personal data, history and background. A full-color photography should be attached (stand-up watching the camera).
    3. An official letter of introduction issued by the board of directors on the letterhead paper of the organization to which the candidate is a registered, active member.

SCHOLARSHIP

A scholarship for this training course program is only available for a registered, active member of a REDES® or MIM-EW® member-association, member-society, member-consortium, or member-fellowship.

Selection Process

The selection committee will evaluate the proposals according to the following criteria:

    1. Fulfilling of all the admission requirements for members.
    2. Nature and extent of experience, particularly in providing the Required Services with institutions and communities in Peru and/or abroad (15 points).
    3. Organization, presentation and content of the proposed training programs as called for in the Required Services Section (50 points).
    4. Proven ability to successfully train persons in measures identified in the Required Services Section, including the use of innovative techniques and/or approaches to meet the unique needs and/or challenges facing institutions and communities in Peru and/or abroad (20 points); and
    5. Ability to enter into and comply with the Standard Contract (15 points).

  • On Friday 1 April 2011 at 12 noon (Hour of Peru).
